Hello, What does "thrown on" mean? I imagine "throw of the dice" kind of means "opportunity" here. The last throw of the dice from Van Gaal sees Marouane Fellaini thrown on in place of Herrera.

Throw of the dice is related to gambling so the manager is taking a chance - here it is the last chance.
To throw on a player (the player was thrown on) means that the team made a substitution though there is a suggestion this was a desperate measure, almost like a last chance for the substitute to make a difference.
